* The original death certificate was marked RETURNED requiring: "Signature of Registrar and date of filing of this certificate." The new one signed by W.W. McVay said: "I issued permit for this on the testimony of G.H. Herlan, undertaker and forwarded the certificate to W.N. Wehr for record. Signed: May 9th 1918."

** The name of the father listed here, BENJ, is in conflict with other records stating it was William Leach.

*** W.T. Newhart could possibly be the husband of his daughter Stella, William Franklin Neuhart. Perhaps the initials are W.F. not W.T. The signature is in the same handwriting as most of the blanks on this form. Other documents show the spelling of his son-in-law as Neuhart.
